Application Description: The iCHOOSE mobile application and corresponding website is expected to serve as a decision support tool for nephrologists, primary care physicians, social workers, nurses, and/or patient educators to use with ESRD patients to help improve informed decision making for kidney failure treatment options. Risk prediction models for mortality on dialysis of kidney transplantation are presented.
July 2018 update: The updated iChoose Kidney now includes mortality and survival risk models based on United States Renal Data System data from 2005 through 2015. Dialysis modality and additional categories for time on dialysis have been added as predictive factors for the new version of the app.
